Key Messages: Vaccine Card Replacement
When you get your first COVID-19 vaccine, you’ll receive a card with the name of the vaccine
and the date. Bring that card with you for any other doses you need, including a booster dose.
The provider will add information about additional doses to the card you already have. If you
lose your card, MDH can’t provide a new one. You can get a new one at your next vaccine
